How to do a – Manual Application
- If you cannot complete your application online, print out the Application /Questionnaire form, complete, then post to us with any other required papers.
- Answer all questions accurately so we have the correct information for your tax refund claim. Incorrect or incomplete answers could delay finalising your refund.
- You will need acceptable photo ID such as a NZ drivers Licence or another acceptable form of photo ID (see list below).
3 Easy Steps
Step 1 – Download the “Application / Questionnaire” form HERE
Step 2 – Complete answers to each question and attach a photocopy of your NZ Driver’s Licence for ID purposes (if you don’t have a valid NZ driver’s licence, select another ID option below)
Step 3 – Sign your application and post to us or alternatively, scan and email it to us.
a) Post the completed application to – P.O Box 317-072, Auckland 0664, NZ – or
b) Scan and email it to – consult@mytaxation.co.nz.
(Make a photocopy for your own records)
Note – If No NZ drivers licence.
If you don’t have a NZ Drivers Licence, we require a copy of another acceptable form of photo ID such as your passport etc (see list below) which must be accompanied by a signed “Authority to Act & ID Verification” form that you can download from the link below.

List of acceptable forms of ID
A) You are over 16.
The types of photo ID that Inland Revenue accepts if you are over 16 yrs old include:
- A valid and current New Zealand Drivers Licence
- A New Zealand Passport (you need to scan/copy the pages that display your photo, name and signature – make sure you sign all copies sent to us)
- A New Zealand firearms or dealers’ licence
- A New Zealand 18+ card
- A New Zealand certificate of identity (issued by the Ministry of Business, Innovation and Employment (MBIE) or Department of Internal Affairs)
- An International Drivers’ Permit (generally issued by a member country of the UN Convention on Road Traffic)
- An overseas passport with New Zealand immigration visa/permit (you need to scan/copy/sign the pages that display your photo, name and signature, any pages showing current work, visitor permits, or residency documentation)
B) You are under 16.
The types of photo ID that Inland Revenue accepts if you are under 16 yrs old include:
- Full New Zealand birth certificate issued on or after 1 January 1998 as they carry a unique identification number. (If you hold a birth certificate issued before 1 January 1998 contact the Department of Internal Affairs if you would like a unique identification number on that certificate)Plus
- Full proof of the parent or guardian identity by providing one fully signed legible scanned copy of an identity document that contains a photo.
